Tramadol – Uses, Dosage, Side Effects & Interactions

Immediate release should be 50 to 100 mg administered orally for pain as needed and should be repeated after 6 hours; however the maximum dosage should not cross 400 mg.

Do note that this dose is individualized and not for patients who need a rapid onset of the effect right away. In such cases instant pain relief sprays should be used instead.

Usual Adult Dose for Chronic Pain:

Immediate-Release is of 2 types in this case, the initial dose and the maintenance dose. For the initial dose, Tramadol 25 mg is given to the patient once a day orally. Thereafter, on reaching 3 days, Tramadol 50 mg is recommended 4 times a day.

The maintenance dose on the other hand has to be titrated first, where Tramadol 50 mg to Tramadol 100 mg is given every 6 hours. The dosage should not cross 400 mg per day.


For an extended release, the Tramadol naive medicine is used titrated at Tramadol 100 mg upwards every 5 days. The dose should not cross 200 mg for each day.

Patients who are receiving immediate release Tramadol are given a 100 mg increment after 3 days using this form of titration in case no benefit is noticed.

Liver Dose Adjustments:

Tramadol is usually inactive in this case and it is not usually suggested as a pain relief measure.

Dose Adjustments:

Do not adjust your dose without consulting your physician.
